Greetings UAW Members,HealthSpan Integrated Care Membership AdministrationPO Box 5388 Cleveland, OH 44101-9372 healthspan.org March 1, 2016 Dear Members: After much consideration, HealthSpan Integrated Care has made the difficult decision to cease offering employer-sponsored healthcare insurance in Ohio. This letter is to notify you of our planned discontinuation of coverage and that effective on midnight on August 31, 2016, your HealthSpan coverage and respective benefits will terminate unless your employer terminates or does not renew coverage with HealthSpan prior to that date. Unless your Evidence of Coverage provides otherwise, HealthSpan will not be responsible for any claims incurred after midnight on August 31, 2016. To help with this transition, we have made arrangements to partner with Medical Mutual of Ohio to provide your employer with similar healthcare coverage options. Medical Mutual can offer coverage that meets or exceeds the product offerings and service you’ve received with HealthSpan. For example, the Medical Mutual provider network spans the same service area you’ve come to enjoy with HealthSpan, And, depending on the plan your employer chooses, it may include many of the same physicians you currently have through HealthSpan. Unless your employer chooses alternative coverage between now and midnight on August 31, 2016, your coverage will continue through August 31, 2016, unchanged according to the terms of our agreement with your employer. We thank you for your membership and trust in us. Enclosed are some frequently asked questions. Please be assured that during the transition period, HealthSpan will continue to provide high-quality healthcare coverage and service to you. Upon request, we will provide you a certificate of credible coverage, which you may need to prove that you have previously been insured.
